我应该选择试管疗程吗?


我见过很多夫妻,结婚超过3年或超过10年都还没能成功怀孕。到底问题在哪里?

年轻的时候可能不着急,没有想要孩子,嘴边总是挂着顺其自然4个字。 到了想要孩子的时候,年纪已经大了,才发现原来怀孕不容易。

有些夫妻就是太太很心急,老公却不当一会事,觉得有没有孩子都无所谓。所以迟迟不愿意配合太太去做检查,导致错过怀孕的黄金期。

有些更是夫妻工作都非常繁忙,或者相隔2岸,就更加没有时间造人。


 

 
当然也有一些夫妻因为迟婚,可能35-40岁才遇上真命天子,所以很迟才能尝试怀孕缺很难怀上,奥莉妈咪本身就是迟婚一族,对于这件事有更深的体会。

女人的排卵,卵子质素和卵子数量会随着年纪增长逐渐下降,所以年轻的女人怀孕成功率比较高,高龄的女人怀孕的成功率就会开始下降。

 


但是很多夫妻往往以上种种原因,导致迟迟无法怀孕。另一方面男人的精子也是不孕因素之一,大部分男人会有畸形精子症,少精子症,甚至有无精子症。无精子症并不是说没有精子,而且他的输精管阻塞了,需要做手术把精子取出来。以上这些问题也是导致无法自然怀孕的原因。只能透过试管疗程来帮助怀孕。

 

那是什么原因男人会出现这种现象,原因很多,部分来自于生活习惯,饮食习惯,抽烟喝酒,熬夜等等。所以如果想计划开始做试管疗程,也需要花时间改变生活和饮食习惯。最好是夫妻一起来调理,改善才会提高出成功率。

所以试管疗程对不孕夫妻来说是一种希望,我更加认为这是一个伟大的技术,因为如果没有这个技术我就无法拥有现在的孩子。试管疗程帮助了很多不孕夫妻成功怀孕,从夫妻2人升级为Family,天伦之乐千金难买。


当然还是很多夫妻犹豫这个问题,成功率高吗?一定成功吗?能一次成功吗?不成功我的钱就没了等等疑问。我个人的看法是这样,如果不尝试,我无法怀孕,所以就算不成功你也没有失去什么。 当然如果成功了是最好。我个人觉得,你要成功,你必须尝试。你不尝试连失败机会也没有。你尝试了,努力过,以后就不会再有遗憾。我是抱着这样的心情去面对。至于金钱方面,每个人的金钱观不一样,有些人认为23万买个包包是值得,买一辆车值得,去一趟豪华旅游更是值得。但是生命无价,花这笔钱,来帮助自己有一个孩子,你又认为值不值?



我看过的夫妻也有很多,有些真的很积极,想奥莉妈咪一样,为了孩子什么都愿意试。也有一些年纪比较高了,卵子数量下降,为了孩子,只能借卵。大家都是想要有自己的孩子,备孕路上做各种的努力,要和自己的丈夫孩子共度天伦之乐,也没有什么不对的。

如何治疗患有严重地中海贫血症的孩子?来了解Saviour Siblings 计划




患有常染色体显性遗传或隐性遗传疾病的夫妇很有可能怀孕具有严重的遗传后果。 β地中海贫血是马来西亚和东南亚流行的一种此类疾病。

总结来说,地中海贫血是马来西亚的公共卫生问题。据估计,这个国家每年有350名婴儿出生,主要是依靠输血治疗地中海贫血。这个数字每年将累积增加,给患者和家庭带来巨大的社会和经济困难。

大多数重型地中海贫血患者都屈服于悲惨,痛苦和折磨的生活。他们的生活以医院环境为中心。定期输血,铁超负荷,全身衰弱和身体不健康损害了他们的生命。失去教育,就业,沮丧和绝望会增加他们的痛苦。


来自健康的HLA相容的骨髓供体或脐带血,优选来自兄弟姐妹的造血干细胞移植(HSCT)可能是β-地中海贫血患者恢复正常血细胞形成(造血)的最佳治疗选择或唯一可能的治疗方法。许多患者仍然无法接受这种治疗,因为难以在直系家庭和大家庭中找到HLA匹配供体。因此,患有非捐赠者的患者和没有捐赠者的患者可以从Savior兄弟姐妹计划(SSP)中受益。

How to Save Your thalassaemia Children with SAVIOUR SIBLING PROGRAMME

SAVIOUR SIBLING PROGRAMME (SSP) A New Lifeline – A Brand New Hope 



Couples with autosomal dominant or recessive disorders have a significant chance of conceiving a pregnancy that has serious genetic consequences. Beta thalassaemia is one such disorder that is endemic in Malaysia and Southeast Asia.

Thalassaemia, in general, constitutes a public health problem in Malaysia. It is estimated that 350 babies are born each year in this country with blood transfusion dependent thalassaemia major. This number will cumulatively increase each year posing enormous social and economic hardships not only to patients and families but also to the government in ensuring optimal care for the existing population of thalassaemia major.


Most of the thalassaemia major patients succumb to a miserable, depressing and painful existence. Their lives are centred around the hospital environments. Regular blood transfusion, transfusion transmitted illnesses, iron overload, general debility and ill-health mar their lives. Loss of education, employment, depression and despair add to their miseries.



Haematopoietic Stem Cell Transplantation (HSCT) from a healthy HLA-compatible marrow donor or cord blood, preferably from a sibling, maybe the best treatment option or the only potential cure for patients with beta-thalassaemia to restore normal blood cell formation (haematopoiesis). Many patients remain unable to receive this treatment because of the difficulty of finding an HLA-matching donor within the immediate and extended family. Thus, patients with nonsibling donors and patient without donors may benefit from Saviour Sibling Program (SSP).


Saviour Siblings are children who are born through in-vitro fertilisation whose tissue (HSC from the cord blood) will be donated so that a brother or sister will be saved from the life-threatening genetic disorder.

Moving beyond routine preimplantation genetic testing for aneuploidy (PGT-A) and preimplantation genetic testing for mutations (PGT-M), Alpha Fertility Centre now offers pre-implantation genetic testing for HLA (PGT-HLA), to help parents conceive a child who is a tissue (HLA) match with their older sibling. During the birth of the saviour sibling, the umbilical cord blood which is rich in stem cells (otherwise considered a waste material) is harvested and infused to the moribund child to restore normal blood cell formation.

Alpha Fertility Centre is the first and only IVF centre in Malaysia that offers NGS-based high-resolution HLA genotyping on embryonic biopsies to select embryos that are chromosomally healthy (PGT-A), disease-free (PGT-M) and HLA matching (PGT-HLA) for the sick sibling. All three genetic tests are done on a single biopsy sample

最新的ICSI 技术PIEZO,为什么要做ICSI ?


一直很想分享这个新的ICSI 技术给大家,
迟迟找不到时间写出来,
今天来给大家讲讲这个

压电式胞浆内单精子注射 (PIEZO-ICSI)

先说说做 ivf 试管,为什么要加ICSI ,原因就是现在很多男人的精子都欠理想,
透过ICSI 胞内精子注射技术,才能更好的让卵子和精子结合在一起。也属于人为的将精子放进卵子中结合。因为如果精子质数欠理想,怎么还可以让精子自己游进去卵子里呢,就是这个原因。


常规式的ICSI 是利用注射针将精子注射进卵子里,这个方式在卵子细胞被注射针穿透 透明带时会变形,
细胞膜穿孔的时机很难拿捏,明显可能伤害卵子细胞。




那么这个新的压电式胞浆内单精子注射 (PIEZO-ICSI)是一种先进的技术,可以减少对卵子的伤害,并于常规式的ICSI相比更能提高受精率。到目前为止,研究表显示电压式ICSI 具有最低的卵子降解率(1%)和最高的受精率(89%)

相对于传统的ICSI,压电式ICSI 使用较细的注射针尖,而注射针尖的顿的而非尖锐的,因此降低伤害卵子的可能性。



研究表明,压电式ICSI 并不影响卵子细胞骨架以及对卵目细胞在细胞分裂时,染色体不受影响。它也表面,从压电式ICSI 衍生的囊胚非整体率是与常规ICSI 类似。
所以选择做ICSI的时候,建议也添加这个PIEZO,费用不多于3百,

关于Day 3/Day 5 胚胎等级的分别

胚胎移植是试管婴儿流程中的一个非常重要的过程,但是很多人可能对于胚胎移植感到比较陌生,特别是有关于试管婴儿胚胎等级的划分,试管婴儿胚胎的质量是影响试管婴儿的成功率的重要因素。


  关于胚胎评级

  目前全世界的试管婴儿诊所胚胎等级划分标准并不统一,一般的AA级囊胚评级非常稀有,而美国好的试管婴儿医院,AA级的囊胚就相对比较常见。另外,有的诊所可能用“811”这样的评级法来描述评定第三天的胚胎,或者用8A来表示。

  卵子(单细胞)

  单细胞发育分裂

  胚胎(Embryo) 

  桑葚胚(morula) 

  囊胚(Blastocyst)

  一般来说,胚胎的移植或是冷冻要么发生在第三天要么发生在第五天。卵子刚取出来,只是单细胞,受精12-24小时后在培养液里发育并分裂,所以到第三天一般是4-8细胞的胚胎(Embryo)。而到第五天,大约发展成上百个细胞,此时胚胎也被称被囊胚(Blastocyst)。界于两者之间(一般是第四天),还有一个阶段叫桑葚胚(morula),因桑葚胚比较难评级,一般不做评级,所以一般胚胎评级都是发生在D3和D5阶段。

  第三天(D3)胚胎评级




  一般来说,胚胎评级跟以下几个因素有关,胚胎的细胞数、分裂后细胞的均匀程度以及碎片化程度。这里先说一个比较常见的分级法,是从一级到四级,一级最好,四级最差,胚胎级别描述:


  一级 细胞分裂均匀; 没有碎片

  二级 细胞分裂均匀; 有少许碎片

  2.5级 细胞分裂大致均匀,有些碎片

  三级 细胞分裂不均匀; 没有碎片或有些碎片

  四级 细胞分裂非常不均匀; 碎片比较严重


  这里要补充说一下为什么会有碎片。当胚胎受到自身原因或体外环境压力时,会产生DNA修复机制,排出对机体有害的细胞成分,导致碎片的生成。所以碎片越多的胚胎,生长潜力可能越差,这成了判断胚胎好坏的一个重要标志。


  一般一级和二级的第三天胚胎都是很不错的,三级的稍差但还可用。四级则不太有被移植的可能性。比如说你被告知第三天的胚胎是一个一级8细胞的,那么恭喜你,成功的可能性还是挺大的。当然,胚胎的好差还跟病人的年纪有关,同样的一级8细胞,年轻女子的染色体变异系数非常低,而年纪大的变异系数却相对高很多。


  有些诊所喜欢用罗马字I-IV来表达第三天的胚胎评级,有些喜欢用A-D来表达。其实参照的描述是差不多的。有些喜欢用三个数字来表示,比如D3的811,第一个数字是细胞数目(8个),第二个数字表达碎片多少(1是几乎没有),第三个数字是表达细胞形态(1为最佳),所以我们可以得知这个811是个几乎完美的第三天胚胎。



第五天(D5)囊胚评级

  根据囊胚腔的大小和是否孵化,将囊胚的发育分为六个时期。



  
  第1期:早期有腔室囊胚,囊胚腔小于胚胎总体积的1/2

  第2期:囊胚腔体积大于或等于胚胎总体积的1/2

  第3期:完全扩张囊胚,囊胚腔完全占据了胚胎的总体积

  第4期:扩张囊胚,囊胚腔完全充满胚胎,胚胎总体积变大,透明带变薄

  第5期:正在孵出的囊胚,囊胚的一部分从透明带中逸出

  第6期:孵出的囊胚,囊胚全部从透明带中逸出

  处于3至6期的囊胚,还需对其内细胞团和滋养层细胞进行质量分级。
  1

  内细胞团分级

  A级,细胞数目多,排列紧密;

  B级,细胞数目少,排列松散;

  C级,细胞数目很少。

  2

  滋养层细胞分级

  A级,上皮细胞层由较多的细胞组成,结构致密;

  B级,上皮细胞层由不多的细胞组成,结构松散;

  C级,上皮细胞层由稀疏的细胞组成。


  三个数字结合起来,就能大致知道囊胚的形态。比如4AA囊胚,它是一个细胞排列紧密、几乎没有碎片、还没有孵出的囊胚,通常被认为最适合移植的囊胚。5AA或6AA,虽然看起来比4AA的更有发育潜能,但因为部分细胞已从透明带逸出,在实质移植操作中反而不如4AA的囊胚更好。


  其实还有D级的分级。但一般到了D级质量就无法移植,所以我们只谈到C级。我们已知道,每个诊所接纳的最低级别不太一样,比如有的接纳的最低级别囊胚是3CC,低于此级别的,则没有移植的意义。此外客人卵子的数量也会影响诊所对囊胚的取舍,有的客户胚胎比较多,3BC这样的级别可能就舍掉不用了,而对卵少的客户来说,一个3BC也许就能活产一个孩子!

  有些胚胎发育慢,到第六天(D6)甚至第七天(D7),才发育成囊胚。因此,除了三个数字以外,囊胚的天数还是判断其发育潜能的重要指标。通常来说,D5的最好,D6的次之,D7的再次之。再晚一点发育的,基本上被认为没有什么发育潜能。D7形成的囊胚虽然慢了点,移植成功率也低了点,但是能挺到这个时候还没有“光荣牺牲”,也已经相当优秀了,所以也不要轻言放弃。

Preimplantation Genetic Testing is already a well-known procedure in many fertility centres, where embryos and/or blastocysts are screened and graded. Only normal embryos will be transferred into the woman’s womb, hence increasing chances of pregnancy and reducing rates of miscarriage. With the ION TORRENT TECHNOLOGY, Alpha is able to run PGS/PGD concurrently on a single platform from a single biopsy. This increases clinical pregnancy rates to 82.9% compared to other systems where the biopsy would be done at least twice, causing possible downgrade to a good embryo/ blastocyst.

EMBRYO FREEZING

The freeze-thaw process is a crucial element in any IVF treatment, where extra oocytes and blastocysts are stored for future use. Since July 2013, Alpha has been using the proprietary CRYOTEC® Freezing Technology for all embryos and blastocysts, as it is the only method with a proven track record of 100% survival rate of embryos and blastocysts during the freeze-thaw process.


PIEZO-ICSI
While most fertility centres currently use Intra-Cytoplasmic Sperm Injection (ICSI) to introduce fertilisation, Alpha is the only centre in SEA that uses Piezo-ICSI. Piezo-ICSI uses a blunt, finer injection tip to introduce the sperm into the egg, hence the possibility of damaging the egg is reduced. This has proven to give better fertilization rate & healthier embryos through monitoring the morphological changes of the embryos.

TIME LAPSE & EMBRYOSCOPY
Alpha has also invested in the capability to visualize the progress of embryo development continuously from Day 0 to Day 7 by taking photographs every 5 minutes in the incubator. This allows embryologists to select only best embryos for transfer and increase IVF success rate.

RESULTS THAT SPEAK FOR THEMSELVES

The centre’s high clinical pregnancy rate of 82.9% in 2016 was presented at the 24th Congress of Obstretrical & Gyneacological Society of Malaysia (OGSM), Kuala Lumpur, Malaysia. At the same conference, Alpha Fertility Centre also presented on a 100% post-thaw survival rates for embryos and blastocysts using the revolutionary Cryotec® freezing method.

A new technique showing that a mix in frozen blastocyst on Day 5 and Day 6 resulted in high clinical pregnancy rates of 71.4% was presented at the 6th Congress of the Asia Pacific Initiative on Reproductive (ASPIRE) in Jakarta 2016.

Another discovery in 2016 – the transfer of a single blastocyst aged less than 120 hours (from insemination to the time of embryo transfer procedure) produced outstanding clinical pregnancy implantation rates of 78.6% – was presented at the 15th International Conference on Preimplantation Genetic Diagnosis (PGDIS), in Bologna, Italy.
